We at the HTCNV pride ourselves on our rapid turnaround times with special form requests. However, it is important to address the HTCNV's official policy in regards to Special Form Requests.
-Special form requests, including FMLA paperwork; travel/school letters; camp physicals; activity clearances; procedure clearances; medical record requests to another provider; etc, shall be submitted at least 10 business days before the date that the form is due.
This allows 10 business days (typically, two weeks) for our staff to complete your paperwork while keeping to normal clinic operations. Submitting form requests in any less time is unacceptable & will not guarantee that we will be able to complete the form you need on time.
